Membership with Seattle Adaptive Sports is required for anyone who participates in three or more practices in a season. All sports programs are included in one (1) yearly membership. All members must have the ability to follow a sequence of instructions and have adequate upper body control.
MEMBERSHIP GIVES ACCESS TO:
- Participation in programs dedicated to adaptive sports for competitive and recreational purposes
- Opportunity to participate in sanctioned events, if the athlete meets selection criteria
- Private Facebook group and emails for communication
- Priority access to community outreach events
By registering, the member agrees to follow the stated Team Policies and Code of Conduct of Seattle Adaptive Sports and the National Governing Body (NGB) of their chosen sport(s).
MEMBERSHIP REQUIREMENTS AND FORMS:
- Complete SAS’s Returning Participant Registration or New Participant Registration or Form*
Steps completed through the Registration Form:- Signing of Move United Waiver and SAS Participant Waiver
- Completion of SAS’s Travel Medical Information Form**
- Payment of $75 Membership Fee
- Register with the National Governing Body (NGB) for your chosen sport(s) and complete SafeSport training

- Participate in Fundraisers and Awareness Events
Once item 1 is confirmed as complete, you will receive a notification that you are a member in good standing. Seattle Adaptive Sports reserves the right to accept or reject any applicant(s) for membership.

**The medical information form is kept in a HIPAA compliant cloud server and will only be available to the coach(es) and medical volunteer of the team(s) the participant is part of. Any questions or concerns about this form can be emailed to nick@seattleadaptivesports.org
