Membership with Seattle Adaptive Sports is required for anyone who participates in three or more practices in a season. All sports programs are included in one (1) yearly membership.

Membership is granted after an athlete or guardian completes and submits their yearly Athlete Information Form and pays the registration fee. Payment can be made via Paypal using the “Pay Registration Fee” icon below or we can accept mailed checks. By registering, the member agrees to follow all rules of Seattle Adaptive Sports and the National Governing Body (NGB) of their chosen sport(s), including a Code of Conduct and Team Policies. 


  • Pay a yearly membership fee of $50, or apply yearly for scholarship 
  • Register with the National Governing Body (NGB) for your chosen sport(s) and complete SafeSport training
  • Participate in fundraisers and special events
  • Complete SAS’s athlete registration form and medical information form*. 
  • A Signed waiver should be emailed to or provided to your coach at the first practice
  • Members must review & agree to the following:
  • All members must have the ability to follow a sequence of instructions and have adequate upper body control.
  • Some equipment is available for loaning.

Please type in the athlete name and sport in the text box under the donation amounts section so we know it is for you/your athlete. We are working to get the PayPal link back up and running as soon as possible.

Upon receipt of your membership dues and a completed yearly athlete information form, you will receive a notification that you are a member in good standing.  

Seattle Adaptive Sports reserves the right to accept or reject any applicant(s) for membership.


  • Participation in programs dedicated to the advancement of adaptive sports for competitive and recreational purposes
  • Opportunity to participate in sanctioned events, if the athlete meets selection criteria
  • Private Facebook group and emails for communication
  • Priority access to community outreach events

* The medical information form is kept in a HIPAA compliant cloud server and will only be available to the coach(es) and medical volunteer of the team(s) the participant is part of. Any questions or concerns about this form can be emailed to
