Membership with Seattle Adaptive Sports is required for anyone who participates in three or more practices in a season. All sports programs are included in one (1) yearly membership.
Membership is granted after an athlete or guardian completes and submits their yearly Athlete Information Form and pays the registration fee. Payment can be made via Paypal using the “Pay Registration Fee” icon below or we can accept mailed checks. By registering, the member agrees to follow all rules of Seattle Adaptive Sports and the National Governing Body (NGB) of their chosen sport(s), including a Code of Conduct and Team Policies.
MEMBERSHIP REQUIREMENTS AND FORMS:
- Pay a yearly membership fee of $50, or apply yearly for scholarship
- Register with the National Governing Body (NGB) for your chosen sport(s) and complete SafeSport training
- Participate in fundraisers and special events
- Complete SAS’s athlete registration form and medical information form*.
- A signed waiver
- Members must review & agree to the following Policies:
- All members must have the ability to follow a sequence of instructions and have adequate upper body control.
- Some equipment is available for loaning.
- If necessary, complete and submit an Equipment Loan Policy & Agreement
- If necessary, complete and submit an Equipment Loan Policy & Agreement
A new registration system is now in place inside the registration forms. This includes options to pay equipment rental fees for the season as well as pay athlete registration fees. Please contact us if the links or any portions of the form are not working correctly!

Upon receipt of your membership dues and a completed yearly athlete information form, you will receive a notification that you are a member in good standing.
Seattle Adaptive Sports reserves the right to accept or reject any applicant(s) for membership.
MEMBERSHIP GIVES ACCESS TO:
- Participation in programs dedicated to the advancement of adaptive sports for competitive and recreational purposes
- Opportunity to participate in sanctioned events, if the athlete meets selection criteria
- Private Facebook group and emails for communication
- Priority access to community outreach events
* The medical information form is kept in a HIPAA compliant cloud server and will only be available to the coach(es) and medical volunteer of the team(s) the participant is part of. Any questions or concerns about this form can be emailed to amorisondc@gmail.com.
